Select a relationship. A nickname and area are optional. Choose “Create my care plan.” You can leave diagnoses and other details blank.
Use “Quick Add” for a next step, appointment or question. Save it, then return to Today. You can fill in the rest another day.
FIND YOUR WAY AROUND
Today
Return here for next steps, upcoming appointments and a quick daily check-in. You do not need to complete every section.
Open Today →My Plan
Collect goals, questions, links and ideas. Start with one action you can realistically take, such as preparing questions for an appointment.
Open My Plan →Schedule
Keep appointment details, questions and what to bring together. Calendar export creates a file to open in your calendar; it is not a live calendar connection.
Open Schedule →About the Person
Record strengths, interests, communication and preferences with the person's involvement. This section is called About Me when planning for yourself.
Open About the Person →Care Team
Keep contacts and plan a handoff. Recording a contact does not send an invitation or give that person access to your plan.
Open Care Team →Records
Organize notes, recorded medicines, document references and other information you choose to keep. Add only what helps your planning.
Open Records →On a phone, Today, My Plan and Schedule are in the bottom navigation. Choose “More” for the other sections. These links open the private workspace and may require sign-in.
Care Plan records are separate from public community posts. They are not published automatically. “Share read-only handoff” lets you choose information for a verified-email recipient; adding someone to your contacts is not the same as sharing.
Review each handoff and any printed or downloaded file before sharing. Include another adult's information only with their agreement or appropriate authority. Review privacy information before storing sensitive details.
Once a week, check the next appointment, update one open step and remove anything no longer useful. A plan can help organize the work; it does not need to become another large task.
Try the Everyday Support Planner or Holiday Plan. Both can be filled in and printed without signing in.
NeuroCoach is an organization and education tool. Saving a question does not contact a clinician; appointments and reminders are not an emergency monitoring service.
