Delaware
Delaware DDDS clients can receive HCBS through the Lifespan Waiver or Diamond State Health Plan Plus. DDDS eligibility and Medicaid waiver eligibility are separate, and a case manager helps determine the appropriate pathway.
Important: Applying to Delaware DDDS is not the same as applying for Medicaid waiver services. Eligible people may need to choose between Lifespan Waiver and DSHP Plus because both cannot generally be used at the same time.
