New Jersey's Division of Developmental Disabilities (DDD) administers two adult Medicaid HCBS programs: the Supports Program and Community Care Program. Support coordinators help participants build service plans around assessed needs and goals.
Important: The Supports Program currently has no waiting list, while the Community Care Program does. Adults generally need DDD eligibility plus Medicaid eligibility/enrollment; Community Care has additional level-of-care and waiting-list requirements.
