New York's Office for People With Developmental Disabilities (OPWDD) is the main state system for developmental-disability services. Care Coordination Organizations help eligible people develop and manage a Life Plan.
Important: OPWDD eligibility and Medicaid/HCBS enrollment are separate steps. Ask about Front Door, care coordination and waiver enrollment rather than assuming one application completes the entire process.
