South Dakota operates four Medicaid HCBS waivers, including CHOICES and Family Support 360 for people with intellectual and developmental disabilities. The Department of Human Services coordinates disability and long-term-support access.
Important: South Dakota's I/DD pathways are split across different waiver purposes. CHOICES provides a broad menu for children and adults with I/DD, while Family Support 360 focuses on helping families support a person at home. Ask DHS which waiver best fits the person's setting and goals.
